Job Description
We are seeking a passionate and enthusiastic individual with professional leadership skills and experience in community nursing. The successful candidate will drive forward best practice patient-centered outcomes, influencing quality and high standards of care.
The post holder will be responsible for strategic leadership, supporting new models and pathways of care within integrated teams and services. They will also contribute to designing and delivering effective care solutions.
Key Responsibilities:

Requirements
To be considered for this role, applicants must have:
* A first-level Registered General Nurse qualification.
* A relevant specialist District nursing qualification.
* Self-motivation and an in-depth knowledge of community nursing criteria, processes, and NHS policies.
