Main ResponsibilitiesConduct screening, assessment, and nutritional follow-up of hospitalized and outpatient patients, providing discharge guidance as requested.
Manage management tools and documents pertinent to the sector (indicators, POPs, among other documents).
Develop and deliver sectoral and institutional training as needed and participate in committees.
Conduct nutritional assessment and follow-up of hospitalized patients based on clinical history, physical and anthropometric assessment, and laboratory tests to formulate nutritional diagnosis, calculate estimated nutritional needs, and define nutritional conduct.
Conduct nutritional assessment of patients on hemodialysis based on clinical history, physical assessment, anthropometric data, biochemical data, among others, to formulate nutritional diagnosis and conduct, documenting data according to institutional norms (using GestHos system and/or sector attachments).
Monthly nutritional follow-up of patients, analyzing biochemical exams, clinical and nutritional status, fluid intake, and other relevant factors, executing necessary dietary interventions and documenting data in accordance with institutional norms (using GestHos system and/or sector attachments).
Supervise nutrition students in the respective sector as necessary.
Daily monitoring of laboratory exams and dietary prescriptions, verifying administered/offered diets.
Participate in bed rounds in ICUs when possible; participate in the Care Plan; discuss clinical cases with medical, nursing, physiotherapy, speech therapy, and other assistance teams (multidisciplinary team); participate in committees; develop and conduct sectoral and institutional training as needed; prepare/update quality management documents; monthly analysis of sectoral indicators; annual analysis of the profile of enteral diets quoted for institutional contract renewal; fill in sectoral spreadsheets post-consultation; document evolution in GestHos for all patients attended, printing documents and filing in physical records; schedule all patients attended in GestHos; monitor dietary acceptance/tolerance, laboratory exams, and clinical evolution of patients; adjust diets according to patient needs; provide nutritional guidance to patients and companions upon discharge; prepare discharge reports and dietary guidance for patients who will use enteral nutritional therapy at home; prepare discharge reports and dietary guidance for specific pathologies upon request; participate in monthly meetings with coordination; print evolutions and attach them to physical records; prepare materials for nutritional education; develop institutional documents and forms as needed; participate in internal meetings, analyzing non-conformities and seeking solutions for quality customer service; fill in care plans for patients with fistulas during the first year of admission and with double lumen catheters; in outpatient care, conduct nutritional assessments of oncology patients based on clinical history, physical assessment, anthropometric data, biochemical data, among others, to formulate nutritional diagnosis and conduct, documenting data in GestHos; maintain patient monitoring by tracking nutritional status, physical assessment, biochemical exams, fluid intake, and other relevant factors, executing necessary nutritional interventions aimed at maintaining and/or recovering nutritional status, documenting all information in GestHos; participate in monthly oncology meetings regarding the therapeutic plan; prepare reports and dietary guidance as needed; conduct appointments for outpatient patients whenever requested by the multidisciplinary team for nutritional care; provide nutritional guidance and follow-up for patients in the nutrition outpatient clinic whenever the ASG-PPP (attachment 01 POP NCL 20) indicates risk (numerical score of 4-8), to ensure continuity of care and promote maintenance and/or recovery of nutritional status; observe safety standards, environmental control, and other internal and/or legal regulations pertinent to activities; participate and suggest improvements in quality management tools, verification and applicability of POP, Risk Management, and Process Mapping; execute other tasks similar to those described above at the manager's discretion; manage and monitor clinical governance of the sectors under responsibility through training management, cost and budget management, people management, results management, team development management, sector risk management, and quality performance management.
